Near the forks of the Black River, Life Care Center of Elyria, Ohio, is less than 3 miles from Elyria Medical Center. With concierge services and an in-house chef, our facility is a perfect place to relax, heal and recover. In June 2017, we completed our annual state survey with no deficiencies.
